Medical Systems has successfully implemented telephalmology
in adult community outreach programs for the detection of
vision-threatening diseases (VTDs). With the alarming increase
in obesity in school children, early case of diabetic retinopathy
among teenagers are on the rise that may lead to vision loss.
With the collaboration of industry- in particular its hardware
and software development programs- the New Jersey Medical
School has initiated a successful school screening program
in an effort to identify students at high risk for diabetic
retinopathy.

Rehospitalizations Reduced Drastically
for Chronically ill Patients by
Utilizing Honeywell HomMed Health Monitoring System
Genesis VNA and Hospice has teamed up with
Honeywell HomMed to demonstrate how utilizing a telehealth
monitoring system will reduce acute care hospitalizations
and bring financial benefits to the agency. Through
an actual case study from the Genesis VNA, attendees will
see how COPD patients have benefited from the use of the Honeywell
HomMed system. Honeywell HomMed has a nationwide network
of partners whose data will also be presented to show the
outcomes and benefits of telehealth on other chronic disease
states.

Matching Technology to Patient Acuity
Levels
The decreased frequency of inpatient admissions for individuals
with chronic conditions is well documented when telehealth
services and other interventions are increased. As a 2005
participant in the QIO Acute Care Hospitalization (ACH) project,
a retrospective review of CMS-generated data identified commonalities
among individuals readmitted to inpatient care while receiving
home health services. Incorporating ViTel Net Inc. technologies,
the project design focused on matching the level of technology
to patient acuity and/ or severity of risk for re-hospitalization.
The presenters will discuss a strategy to successfully realign
care, costs and outcomes for individuals with a diagnosis
of heart failure.

Physician Driven Chronic Care Management
Moving to a new model of preventive care for individuals
with severe, complex chronic conditions is perhaps the biggest
challenge currently facing health care providers and payers.
The Centers for Medicare and Medicaid are meeting this challenge
by contracting with select programs to carry out demonstration
projects for beneficiaries with complex illness. This session
will introduce the Health Buddy Program, one such demonstration
project designed to show how medical groups -- with the right
incentives and technology -- can help patients with severe
chronic conditions lead healthy, happy lives at a dramatically
reduced cost.

Medication Adherence
Case
Studies that Demonstrate the Most Promising Strategy to Improve
Quality and Reduce the Cost of Care Case Studies utilizing
new technologies will be presented that promote and monitor
medication and care plan adherence in the home. Outcomes document
adherence rates greater than 90% in multiple centers across
various disease states and significant improvement in biological
markers (ex. HbA1C). Based upon review of adherence literature,
reductions in cost of care and improvements in quality of
care associated with improved medication adherence will be
presented. This telemonitoring solution addresses improving
adherence and, improving patient safety, (by aiding the patient
with complex drug regimens through programmed reminders and
daily monitoring) and thus improves quality and cost combined.

off the Ground with Telehealth – A
Home Health Agency's Experience with Readiness

TeleHealthcare was chosen by the Visiting Nurse Association
of Albany, Inc., (VNA) a New York State Certified Home Health
Agency, to provide home telehealth monitoring for their patients
with Congestive Heart Failure. The VNA program has been in
place for less than a year, but the time invested before program
implementation was critical to its internal acceptance and
success. Discussion will include: VNA staff involvement and
buy-in as an essential element of a new service provision;
pre and post patient and VNA staff participation survey results;
orientation to the equipment and program; and initial marketing
efforts

What a Successful Program Looks Like
Of the hundreds of Telemedicine programs that have
been launched across this country over the past 10 years,
one of the most successful is sponsored and administered by
Blue Cross of California. Focused initially on Teledermatology,
it is now expanding to many other specialties. Using Store
and Forward software and live video, the program has conquered
the issue of distance from a rural site to a specialist. More
impressive, it has cut the wait time for appointments from
100+ days (the usual period for Medicaid patients) to less

Improving
Asthma Control with Telemedicine Technology
Monitoring
of lung function through assessment of asthmatic patients'
peak flow trends is a well recognized tool in this disease's
management. Consequently peak flow monitoring remains a mainstay
in asthma clinical research trials and study outcomes. The
National Asthma Education and Prevention Program (NIHI) suggests
that asthma management should strive to be a collaborative
effort between patients and their physicians or primary care
providers. As the impact of this disease remains an important
healthcare issue, how might this collaboration be facilitated?
Advantages for research and disease management of a system
of electronic lung function monitoring linked with web-based
trending & viewing functionality are discussed.
